A case of gangrene of Great Toe

An 84-year-old male was brought to my consultation with complaint of pain in great toe of right foot with swelling and since last 2 months.
As the patient entered our consultation there was a fetid odor around the cabin.
Soon we realized that the wound may be gangrenous and then we suggested for an arterial Doppler to look for obstruction and development of gangrene.
The arterial Doppler was conclusive as expected, there was 70 % luminal narrowing and the gangrene was developing rapidly in the toe.
After the Doppler studies we took the case in detail and then prescribed him a medicine for 15 days with a great positive hopes. 
The patient showed us again after 15 days. 
Amazing! The 1st thing that was gone is the fetid odor, which was an indication that the gangrene was recovered.
The pain was gone and so was the swelling.
The photo will elicit that during 1st consultation the swelling in great toe was too much with drastic increase in size, where one cannot differentiate between 1st and 2nd toe. Whereas in the follow up we can differentiate between the 1st and 2nd toe.
